Information the Customer Should Complete
If you’re bringing your own FD-258 card, please complete the following before you meet the fingerprint examiner.
Personal Information (Top of Card)
- Name — Last, First, Middle (print clearly in the boxes)
- Aliases — Any other names used (including maiden name)
- Date of Birth — MM/DD/YYYY
- Citizenship (CTZ) — Country of citizenship
- Sex — Male, Female, or Unknown
- Race — Use standard FBI race codes
- Height — In feet and inches
- Weight — In pounds
- Eye Color — BLU, BRN, GRN, GRY, HAZ, BLK, etc.
- Hair Color — BLN, BRN, BLK, RED, GRY, WHI, etc.
- Place of Birth — State or Country
- SSN — If required by the requesting agency
- MNU — Any other identifying number (if applicable)
Agency / Reason Section
- ORI (Contributor) — Originating Agency Identifier (if known)
- Date — Today’s date
- Employer & Address — If employment-related
- Reason Fingerprinted — Employment, Licensing, etc.
- Armed Forces # — If applicable
- FBI # — Leave blank (assigned by FBI)
- State ID # — If known
Important Notes
- Use black or blue ink only
- Print clearly in CAPITAL letters; use standard abbreviations only
- Do not write in any fingerprint blocks — the examiner will complete these
- Make sure all info matches your government-issued ID
- Do not sign the card unless the examiner instructs you to
